Job Description
The US Registered Nurse (USRN) reviews medical requests to make sure services are medically necessary and aligned with clinical guidelines. This role works closely with doctors, care teams, and insurance partners to support safe, appropriate, and cost‑effective patient care.
Key Responsibilities- Provide telephonic and virtual patient assessments, care coordination, and ongoing monitoring for assigned patient populations.
- Deliver patient and family education related to conditions, treatment plans, medications, and self-management strategies.
- Develop, implement, and update individualized care plans based on clinical guidelines and patient needs.
- Monitor patient progress, escalate clinical concerns appropriately, and support early identification of deterioration or barriers to care.
- Coordinate services across providers, payers, and community resources to ensure continuity of care and adherence to care plans.
- Document all interactions accurately and promptly in electronic health records (EHR) and care management platforms.
- Collaborate with clinical and operational team members to optimize workflows and patient engagement.
- Has active PHRN license
- Has active USRN license (Mainland States preferably New York License but not required)
- Has at least 2 years DIRECT CARE experience / hospital bed side experience in ICU, Telemetry, Emergency & Medical Surgical Department
- Has at least 1 year of Utilization Management Review Experience (preferred for other accounts**)
- Good to Excellent Communication Skills
- Can start ASAP
- Must be amenable to work ONSITE (Site Locations: Ortigas, Taguig, Alabang)
Salary Offer : 55,000 to 70,000 salary package (can be negotiable depending on the account and years of experiences)
