Caregiver (institution-based)

Job Description

1. Takes the vital signs of the patients and report irregularities to Nurse on Duty.
2. Assists Nurses during IV insertion, catheterization, wound dressing, NGT insertion, suctioning and does the after care.
3. Prepares the rooms for admissions in coordination with housekeeping- on- duty and does bed making.
4. Changes linens every morning and brings the soiled linens to Laundry area, removed soiled linens upon discharge
5. Assists/ accompanies patients in various activities:

• going to other concern departments
• during discharge
• going to their room of choice

1. Receives endorsement of linens, equipment, supplies, and activities done or to be done to patients.
2. Attends during CODE 98 and performs ambubagging, CPR, monitoring of vital signs.
3. Maintains cleanliness and orderliness of the whole unit in coordination with housekeeping-on-duty.
4. Run errands:

• forwards requests to different departments.
• gets medicine from Pharmacy.
• replaces supplies from CSR and Purchasing.
• submits schedules to HR department.
• brings specimens to Laboratory.
• forwards discharge notification to Billing Section.

1. Fills checklist of furniture and fixtures at patients room prior to discharge.
2. Makes necessary charges for certain nursing procedures done.
3. Sees to it that used articles and supplies are replaced.
4. Makes appropriate reports to Head Nurse.
5. Does post mortem care.
6. Assists patients in Hygienic measures.
7. Photocopying needed documents.
8. Measures intake and output of every shift and have the knowledge to identify patients’ conditions that needs referral.
9. Performs other applicable tasks/duties as assigned from time to time within the realm of knowledge, skills and abilities.
10. Answers patients’ calls and delivers message to assigned Nurse.
11. Checks availability of supplies for Ward use, makes sure that all equipment issued are functioning and complete.
12. Cleans patient’s charts and equipment at least twice a month.

Qualifications/Requirements

graduate of caregiving nc2, underboard nurse or practical

preferably male
