About the Role
We are hiring a Healthcare Intake & Enrollment Specialist to support a U.S.-based dementia care program under the CMS GUIDE Model initiative.
This role focuses on referral intake, caregiver outreach, appointment scheduling, patient engagement, eligibility review, and enrollment coordination.
The ideal candidate is compassionate, organized, confident on calls, and experienced in healthcare intake, scheduling, customer service, or patient coordination.
Key Responsibilities- Receive and track incoming referrals
- Conduct caregiver outreach and intake calls
- Explain the healthcare program clearly and professionally
- Schedule intake appointments and coordinate follow-ups
- Maintain accurate referral documentation and tracking
- Assist with eligibility screening and compliance checklists
- Coordinate with operational and healthcare teams
- Reduce no-shows through appointment reminders and follow-up
- Experience in healthcare intake, scheduling, patient coordination, customer service, or medical administration
- Strong English communication skills
- Comfortable speaking with caregivers and older adults
- Organized and detail-oriented
- Experience using CRMs, spreadsheets, scheduling systems, or EHRs
- Healthcare sales, enrollment, or call-center experience is an advantage
- Onsite work setup
- Office located in Bacoor, Cavite
- Schedule and deployment date to be announced
