Job Description
Posted on 24 April 2026- Receives nursing reports from the incumbent shift
- Performs patient assessment (head to toe) and apply the nursing process in dealing or attending to patient needs such as assessment, diagnosis, planning, intervention and evaluation.
- Performs non invasive hemodynamic monitoring
- Admits patient and helps the nursing assistant on the preparation for the admission such as setting up the cardiac monitors, infusion pumps, suction machines and bedside needs
- Collaborates with the doctors on the intervention needed by the patient
- Makes referrals on patient ever changing condition to ACOD’s and AP’s
- Gives medication as ordered and make medication cards
- Makes request to different departments like lab requests, radiology request, heart station and pulmonary as ordered by the physician.
- Conducts continuous assessment and monitoring of patient and do critical judgment
- Documents all procedures done, assessment and intervention according to acceptable standard of nursing practice
- Carry out doctors order of attending physician
- Does intravenous insertion as IV therapy nurse
- Does various nursing procedures such as suctioning
- Monitors vital signs of patients and input and output and refer if needed
- Secures waivers for DAMA and DNR on a daily basis after proper explanation was done.
- Discharge patient according to hospital policy and complete discharge notification instruction and makes necessary charges to patients
- Helps in setting up ventilators and do adjustment on its settings as ordered
- Performs peritoneal dialysis and handle post kidney transplant patients, post craniotomy patients, mechanically ventilated patients, cardiac patients and the like.
- Attends to CODE 98 and performs nursing functions as assigned.
- Coordinates with other department like admitting, other stations on patient transfer or discharge
- Gives nursing reports to receiving dept. and assures patient safety during transfer
- Completes the needed forms
- Compliance on hospital policies and procedures and programs
- Coordinates with ACOD’s when patient request for transfer to other institution
- Prepares documents needed for transfer
- Assists the doctor in monitoring the patient during conduction
- Performs post mortem care or baptism to unbaptized neonates
- Informs guard on duty on transfer of cadaver to morgue
- Logs information in the corresponding logbook like admission, discharge and request
- Arranges charts according to medical records policy and submits the charts to admitting dept/benefits section
- Makes ambulance arrangements
- Establishes communication between the patient and the persons with whom the patient comes into contact
- Gives update to the family members on the patients status in scope of nurses
- Provides spiritual support when necessary
- Maintains unit records on necessary clinical and other data requested for statistical analysis
- Maintains confidentiality in patient, family, staff and all hospital matters
- Establishes good working relationships with all other employees on the unit and in the hospital
- Ensures that all unit policies and procedures and work instructions are current and readily available
