Medical Social Worker

Job Summary

The Medical Social Worker provides social services to patients and their families to help them cope with the social, emotional, and financial aspects of illness and hospitalization. The Medical Social Worker assists in accessing available resources and coordinates with hospital staff and external agencies.

1. Conduct psychological assessment of patients and families to determine social, emotional and financial needs.
2. Provide counseling and support to patients and their families during hospitalization and treatment.
3. Assist patients in securing financial assistance from programs such as Philhealth, PCSO, DSWD, LGU and other charitable institutions.
4. Coordinate with physicians, nurses, and other healthcare professionals regarding patient care and discharge planning.
5. Facilitate referrals to appropriate government agencies, community resources, and support organizations.
6. Advocate for the welfare and rights of patients, particularly indigent and vulnerable individuals.
7. Maintain accurate records of social service interventions, patient assistance, and referrals.
8. Prepare required reports and documentation in compliance with DOH policies and hospital regulations.
9. Participate in hospital programs, case conferences, and multidisciplinary team meetings when necessary.
10. Perform other duties assigned by hospital management related to social services and patient support.

• Bachelor's Degree in Social Worker
• Licensed Social Worker in accordance with the Professional Regulation Commission (PRC).
• Preferably with experience in hospital or healthcare setting.
• Knowledgeable in government assistance programs and hospital social service procedures.
• With good interpersonal, counseling, and communication skills.
