Job Description
- Conducts pre-admission planning and psychosocial assessments.
- Joins general rounds and monitors patient stay and billing status.
- Coordinates with doctors, nurses, and other staff to ensure holistic patient care.
- Assists patients/representatives in resolving issues during ER stay.
- Mobilizes resources to support patient medical needs.
- Prepares referrals and communications for coordination with charities and community agencies.
- Conducts limited home visits and liaises with welfare organizations to address patient needs.
- Bachelor’s Degree in Social Work
- Must be a Licensed Social Worker under PRC
- Hospital experience is an advantage
- Strong skills in case study preparation and documentation
