This role is critical in ensuring clinical documentation accuracy, regulatory compliance, and overall survey readiness, while providing direct support to leadership in maintaining smooth day-to-day operations.

This position is ideal for a highly experienced nurse who can operate independently, take ownership of QA processes, and deliver reliable, detail-driven work. In addition to core QA responsibilities, the role will provide light operational support, including oversight of scheduling and authorization tracking, within a fast-paced and expanding environment.

Key Responsibilities

Clinical QA & Compliance
  • Review clinical documentation for accuracy, completeness, and compliance
  • Perform ongoing chart audits to ensure adherence to Medicare, Medicaid, and VA guidelines
  • Identify documentation deficiencies and communicate corrections to clinical staff
  • Support survey readiness and accreditation preparation
  • Maintain audit-ready records and ensure regulatory compliance at all times
  • Assist in developing and maintaining QA processes and workflows
  • Monitor documentation timeliness including visit notes, orders, and certifications
  • Ensure EMR (Axxess or similar) documentation is accurate and up to date
Executive & Operational Support
  • Serve as a clinical executive assistant to the owner and Director of Nursing
  • Provide high-level support to relieve clinical leadership workload
  • Assist with tracking and managing authorizations to ensure timely approvals
  • Review schedules to ensure alignment with authorized services and patient needs
  • Support coordination between clinical, intake, and administrative functions
  • Help monitor operational workflows and flag inefficiencies or gaps
Additional Support Functions (Light Duty)
  • Assist with scheduling oversight and adjustments as needed
  • Support authorization tracking and follow-ups
  • Collaborate with intake and coordination teams as needed
  • Take on additional administrative or clinical support tasks as the agency grows
Preferred Skills & Qualifications
  • Active RN license required
  • Minimum 3+ years of home health experience (required)
  • Strong Quality Assurance / chart audit experience in home health
  • Experience with Axxess EMR or similar system required
  • Strong understanding of Medicare, Medicaid, and/or VA home health requirements
  • Experience with survey readiness and accreditation processes
  • Ability to work independently with minimal training or supervision
  • Strong attention to detail and organizational skills
  • Excellent communication and problem-solving abilities
Nice-to-Have (Bonus Skills)
  • Experience working with VA or veteran patient populations
  • Prior experience in a QA or compliance-focused role
  • Experience supporting fast-growing home health agencies
  • Familiarity with multi-service lines (SN, PT, ST, etc.)
  • Experience in an executive assistant or leadership support capacity
