Job Description:
Purpose
The Intake Coordinator serves as the first point of contact for individuals seeking HCBS services. This role is responsible for
managing the intake process, conducting assessments, verifying documentation, and coordinating with case managers and
healthcare professionals. The Intake Coordinator ensures compliance with state and federal regulations and maintains
accurate client records while providing support to clients and their families.
Key Responsibilities
Client Intake and Assessment
Serve as the first point of contact for individuals seeking HCBS services.
Conduct intake assessments to determine eligibility and service needs.
Provide information to clients and families regarding available services and program requirements.
Assist with scheduling initial assessments and onboarding new clients.
Documentation and Compliance
Collect and verify required documentation, including medical and financial records.
Maintain organized and up-to-date client records in the system.
Ensure compliance with state and federal regulations for HCBS programs.
Adhere to HIPAA and other regulatory compliance requirements.
Coordination and Communication
Coordinate with case managers, healthcare professionals, and community resources.
Work collaboratively with the care team to develop individualized service plans.
Address client inquiries and resolve concerns professionally and promptly.
Reporting and Support
Monitor and track intake progress, following up on pending cases.
Assist in resolving any issues or concerns related to the intake process.
Participate in training sessions and team meetings to stay updated on best practices and process improvements.
Required Knowledge and Skills
Intake and Client Services Expertise
Experience in intake coordination, medical billing, Appointment Setter, healthcare administration, or a related field.
Knowledge of medical terminology, insurance processes, and ABA/HCBS program requirements.
Ability to research discrepancies, reconcile documentation, and ensure data accuracy.
Communication & Organizational Skills
Strong ability to interact with clients, providers, and payers while managing multiple tasks efficiently.
Excellent verbal and written communication skills.
Strong organizational and time-management skills.
Education & Experience
College Level or equivalent required; Associate’s or Bachelor’s degree in a related field preferred.
Experience in intake coordination, revenue cycle management, or healthcare administration.
Prior experience in ABA/HCBS, Behavioral Health, or therapy-based services is preferred.

Proficiency in using electronic health records (EHR) systems.
