Pharmacist

The Pharmacist will monitor drugs prescribed by physicians and other health practitioners and provide information to patients about medications and their use. He or she may advise physicians and other health practitioners on the selection, dosage, interactions, and side effects of medications.
- Manage and review patients’ medication therapy to ensure safety and effectiveness
- Perform medication reconciliation and resolve prescribing issues
- Support antibiotic stewardship by monitoring use and ensuring guideline compliance
- Educate patients on medication use, side effects, and adherence
- Monitor and maintain crash carts, ensuring completeness and compliance
- Collaborate with healthcare providers on patient care plans
- Provide clinical support, including therapy recommendations and drug interaction management
- Assist in immunization and preventive care services
- Participate in quality improvement initiatives and clinical research
- Support policy development and ensure regulatory compliance
- Provide general customer care and service
Qualifications:
- Graduate of BS Pharmacy
- Licensed Pharmacist
- At least 6 months of experience in a similar capacity, preferably in a healthcare or clinic setting.
- Proficiency in basic computer operations
