• Audit OASIS documentation, including SOC (Start of Care), ROC (Resumption of Care),

• Review clinical notes and ensure necessary corrections have been made before final approval.
• Approve corrected OASIS entries and ensure documentation aligns with Medicare and PDGM

• Enter any missing medications based on referral documents.
• Create CMS 485 / Plan of Care and generate required physician’s orders.
• Conduct OASIS checks and perform detailed analysis of documentation quality.
• Audit Skilled Nurse Visit (SNV) notes and send back corrections to clinicians as needed.
• Collaborate with clinicians to maintain documentation accuracy and regulatory compliance.

• Registered Nurse (RN) with a valid nursing license.
• Degree in Nursing (Associate or Bachelor’s degree).
• In-depth knowledge of Medicare guidelines, PDGM, and ICD-10 coding.
• Strong grasp of clinical documentation standards, quality assurance practices, and regulatory

• Ability to work independently with minimal supervision, as well as collaboratively within a team.
• Excellent communication, interpersonal, and analytical skills.
• Prior experience in home health QA reviewing or clinical auditing is a plus.
