Remote Clinical Documentation Nurse

(Philippines – Night Shift)

About the Role

A US-based home health company is building a team of remote Registered Nurses in the Philippines to support its clinical documentation program.

In this role, you will review US patient charts, identify documentation gaps, and help ensure accurate medical records and coding — all under the supervision of a licensed US-based nurse.

This is a non-patient-facing role focused on documentation and compliance.

• Review clinical charts and medical records for accuracy and completeness
• Identify gaps in physician documentation impacting diagnosis and procedure coding
• Support medical coding validation (ICD-10, CPT, HCPCS) under certified coders
• Conduct retrospective and concurrent chart audits to prevent compliance issues before billing
• Provide virtual scribing support during scheduled telehealth consultations
• Submit findings and recommendations in the required format to the US supervising nurse
• Respond promptly to feedback and follow-up queries
• Maintain strict confidentiality of all patient health information

Requirements:

• Active PRC Nursing License in good standing
• Minimum 2 years of clinical experience (hospital, clinic, or home health)
• Strong English communication skills (written and spoken)
• Working knowledge of medical terminology, ICD-10, and CPT coding
• Reliable home internet (minimum 25 Mbps upload & download, verified via live test)
• Availability to work US time zones (fixed night shift, Philippine time)
• Comfortable using EHR/EMR systems or able to learn quickly
• Quiet home workspace with a working webcam and headset

• Experience in CDI, chart auditing, medical coding, or transcription
• Certifications such as CDIP, CCS, or CPC
• Previous remote work or healthcare BPO experience
• Familiarity with US healthcare standards (CMS, Joint Commission)
• Experience using telehealth platforms
