Overview
We are seeking an experienced Clinical Quality Assurance & Operations Nurse to support quality assurance, clinical documentation review, care coordination, and operational workflows across the organization.
This role is designed for a Registered Nurse who brings strong home health experience and can assist with chart audits, documentation review, compliance support, intake coordination, scheduling, and authorization workflows. The ideal candidate combines sound clinical judgment with strong organizational skills and is comfortable working across both clinical and administrative functions.
This position is not intended to function as a standalone QA Manager or Compliance Director. Instead, it serves as a hands-on support role that helps ensure documentation accuracy, operational consistency, and timely patient care coordination while partnering closely with clinical leadership and quality assurance team members.
Key Responsibilities
Clinical Quality Assurance & Documentation Support- Conduct routine chart reviews to support documentation accuracy and compliance.
- Review clinical records, physician orders, care plans, visit documentation, and supporting records for completeness and consistency.
- Identify documentation discrepancies, missing information, and areas requiring follow-up.
- Assist with quality assurance reviews and audit activities under the guidance of leadership.
- Support ongoing quality improvement initiatives and documentation best practices.
- Track quality-related findings and ensure appropriate follow-up occurs.
- Help maintain documentation standards that align with organizational and regulatory expectations.
- Collaborate with clinicians and leadership to resolve documentation questions and concerns.
- Review patient records for accuracy, completeness, and timely documentation.
- Assist with tracking outstanding physician orders and documentation requirements.
- Follow up on missing, incomplete, or conflicting clinical information.
- Support medication reconciliation and documentation clarification efforts when needed.
- Help ensure patient records remain organized and audit-ready.
- Review incoming referrals and supporting documentation for completeness.
- Assist with intake coordination and admission readiness activities.
- Support insurance authorization tracking and follow-up.
- Communicate with referral sources, physicians, and internal staff regarding missing information.
- Assist with resolving referral and authorization-related bottlenecks.
- Help ensure patients move efficiently through the admission process.
- Assist with clinician scheduling and coverage coordination as needed.
- Support schedule adjustments related to staffing needs, call-outs, or patient care requirements.
- Help identify operational issues that may impact patient care or workflow efficiency.
- Maintain organized tracking systems to support accountability and task completion.
- Assist leadership with day-to-day operational coordination and follow-up activities.
- Active Registered Nurse (RN) license required.
- US RN licensure preferred.
- US home health experience required.
- Experience with quality assurance, chart audits, documentation review, or compliance support.
- Familiarity with home health intake, referrals, scheduling, and authorization workflows.
- Strong understanding of clinical documentation standards and home health operations.
- Ability to identify documentation inconsistencies and follow issues through resolution.
- Strong organizational skills with excellent attention to detail.
- Effective written and verbal communication skills.
- Ability to manage multiple priorities in a fast-paced environment.
- Comfortable working independently while collaborating with clinical and operational teams.
- OASIS review experience.
- Experience supporting quality assurance or clinical audit programs.
- Experience mentoring or assisting other clinical team members.
- Familiarity with Medicare home health documentation requirements.
- Experience supporting high-volume home health agencies.
- Background in workflow improvement or operational support initiatives.
- Experience working across both clinical and administrative functions.
