JOB QUALIFICATIONS:
- Must be a graduate of Bachelor's degree in Social Work or related program/field
- Must be a licensed and registered Social Worker
- Preferably with at least three (3) years of experience in Medical Social Work or related roles in the hospital/healthcare setting
- Preferably with related work experiences or exposure to Kidney Transplant cases
MAJOR JOB FUNCTIONS:
- Responsible for overseeing the day-to-day operation of the Kidney Transplant Center's Social Service unit
- Provide overall coordination and management of medical social work programs
- Conduct patient case assessment and support facilitation
- Direct social work services in various clinics and treatment area
- Manage appropriate assignment of clients and ongoing clinical care, and monitor the statistics and profile of clients utilizing the clinic for treatment
- Perform other functions as may be assigned by immediate superior from time to time.
