Quick Rundown:
We are seeking a detail-oriented and customer-focused Intake Coordinator to support patient intake, referral coordination, and authorization processes for Home Health services. This role is responsible for managing referrals, verifying patient eligibility, coordinating with healthcare teams, and ensuring smooth patient transitions while maintaining excellent customer service and compliance standards.
What You’ll Do:
- Manage patient referrals, admissions, and intake coordination for Home Health services
- Gather and verify patient information, insurance coverage, authorizations, and eligibility
- Coordinate with physicians, case managers, social workers, and internal teams regarding referrals and patient care transitions
- Communicate with patients, families, and caregivers regarding services, coverage, and scheduling
- Monitor referrals, documentation, and service utilization to ensure timely processing and compliance
- Answer office phones and provide support for inquiries and scheduling needs
- Maintain referral tracking reports and update patient information in ABS and internal systems
- Assist with issue resolution, escalations, and coordination between departments
- Support community and partner relationships by providing timely updates and excellent service
What We’re Looking For:
- 2+ years of relevant experience preferred
- Experience in healthcare intake, scheduling, customer service, or home health is a plus
- Strong communication, organizational, and multitasking skills
- Excellent phone etiquette and customer service orientation
- Ability to work independently and manage urgent priorities effectively
- Proficient in Microsoft Office (Word, Excel, Internet applications)
- Strong attention to detail and problem-solving abilities
- Comfortable handling multiple referrals, scheduling demands, and coordination tasks in a fast-paced environment
- Familiarity with Home Health processes, insurance verification, and authorizations
- Experience using ABS or similar healthcare systems
- Ability to troubleshoot issues and escalate concerns when necessary
