As a Staff Pharmacist (In-Patient), you will play a critical role in ensuring safe, accurate, and timely medication management for hospitalized patients. This position requires strong clinical knowledge, attention to detail, and collaboration with healthcare teams to optimize patient outcomes.
Key Responsibilities:
- Medication Management: Review, prepare, and dispense medications for in-patient use in compliance with hospital protocols and regulatory standards.
- Clinical Support: Provide drug information and therapeutic recommendations to physicians and nurses; monitor patient medication therapy for efficacy and safety.
- Inventory Control: Maintain accurate stock levels, manage medication storage, and ensure proper handling of controlled substances.
- Patient Safety: Conduct medication reconciliation, prevent drug interactions, and participate in adverse drug event reporting.
- Documentation & Compliance: Ensure accurate record-keeping in the hospital system and adherence to all legal and accreditation requirements.
- Collaboration: Work closely with doctors, nurses, and other departments to support patient care plans.
- Must be a graduate of Bachelor of Science in Pharmacy
- Candidate must have a valid PRC License.
