Key Responsibilities:

• Facilitate patient intake processes and ensure accurate, timely documentation of medical information
• Coordinate care activities across multidisciplinary healthcare teams to support seamless service delivery
• Monitor patient care plans and track progress, outcomes, and adherence to treatment protocols
• Contribute to clinical quality improvement efforts by identifying gaps and supporting process enhancements
• Managing appointments, coordinating referrals and help in the healthcare system navigation

• Educate patients on care plans, procedures, and available treatment options to promote informed decision-making
• Respond to patient inquiries regarding services, scheduling, and care coordination
• Act as a liaison between patients and healthcare providers to ensure clear, consistent communication
• Conduct follow-ups to assess patient needs, address concerns, and support overall satisfaction and continuity of care
• Work closely with the patient's family in handling concerns and provide support throughout the treatment process.
